Applicant with a Medical Condition [ + ]

• The applicant for a temporary permit is asked the medical status questions on the application. The following procedures must be met if the applicant states that he/she has a medical condition that possibly could or would not affect his/her driving.
1. Medical form BMV 2310A is issued by examiner.
2. Applicant must return completed medical form to exam station within 30 days.
3. Examiner retains the exam form.
4. Examiner mails (form BMV 2003) (to include vision and written scores) to BMV in 2341 envelope.
• Applicant returns medical form to examiner who checks form to determine if it has been completed and contains a physician's statement. If form is not completed in its entirety, the examiner returns the form to the applicant to have it completed. See completion of medical packet.

Applicant Converting a Valid Out-of-State License [ + ]

• After completing the vision and written examinations applicant is asked medical status questions. The procedures outlined above will be followed if the applicant states that he/she has a medical condition that possibly could or would not affect his/her driving. Since the applicant had been the holder of a valid out-of-state license, one additional procedure is added: Applicant is given a form BMV 2015 (provides driving privileges within Ohio for 60 days).
• Applicant returns medical form to examiner who checks form to determine if it has been completed and contains a physician's statement. If form is not completed in its entirety, the examiner returns the form to the applicant to have it completed. See completion of medical packet.

First-time applicants must do the following:

Go to a Driver Examination Station (DX) [ + ]

• Take documentation to provide evidence of your full legal name, date of birth, and social security number (if one has ever been assigned), proof of legal presence (U.S. Birth Certificate, valid U.S. Passport/Passport Card, Naturalization Papers, USCIS documents), and proof of residency. See Acceptable Documents.
• Take a vision and a written examination.
• Answer questions concerning your physical health and any medical conditions you might have. If you have a condition that might affect your driving, the examiner will explain the steps you have to take and provide you with a medical packet (form BMV 2058) which must be filled out by your doctor. Pass the written and vision tests, then do the following: [ + ]

• Apply for a Temporary Instruction Permit Identification Card (TIPIC) at your local Deputy Registrar.
  See Driver License Fees. Take with you documentary proof of your full legal name, date of birth, Social Security number, (if one has ever been assigned), proof of legal presence (U.S. Birth Certificate, valid U.S. Passport/Passport Card, Naturalization Papers, USCIS documents), and proof of residency.  See Acceptable Documents.
• If you are under 18 years of age, you must be accompanied by a parent or guardian, or person having legal custody, or, if there is no parent or guardian, a responsible person who is willing to assume the obligations imposed under Rule 4501:1-1-19 of the Ohio Administrative Code. The co-signer must also take documentary proof of his identification. See Acceptable Documents.

• Once the TIPIC is issued, you will be able to practice driving but only if you are accompanied by a licensed driver in the seat next to you.

Ohio Administrative Code Section 4501:1-1-08

An applicant must pass a written test that is given in two parts and covers Ohio motor vehicle regulations and traffic signs.

Motor Vehicle Regulations [ + ]

• Minimum of 20 multiple choice questions equally weighted.
• Must correctly answer 75% or 15 questions to pass this portion of the test.
• Questions cover: Motor vehicle laws; rules of the road pertaining to safety; regulations of the BMV as prescribed by the director of public safety.

Traffic Signs [ + ]

• Minimum of 20 multiple choice questions equally weighted.
• Must correctly answer 75% or 15 questions to pass this portion of the test.

• Questions cover: Highway warnings and traffic signs and signaling devices and directions given in the English language.

CDL Written Test [ + ]

• Minimum of 50 questions equally weighted.
• Must correctly answer 80% or 40 questions to pass this portion of the test.

• Questions cover: General commercial driver knowledge

Qualifications for Special Endorsements: [ + ]

• Must correctly answer 80% in each specific category as follows to pass:
1. Combination Vehicles:
   20 questions, need 16 correct answers
2. Double or Triple Vehicles:
   20 questions, need 16 correct answers
3. Hazardous Materials:
   30 questions, need 24 correct answers
4. Passenger Transport:
   20 questions, need 16 correct answers
5. Tankers:
   20 questions, need 16 correct answers
6. School Bus:
   20 questions, need 16 correct answers
7. Passenger Transport:
   20 questions, need 16 correct answers

   NOTE: The hazardous material test can never be taken orally.

Air Brakes [ + ]

• Must correctly answer 80% or 20 questions to pass this portion of the test.

The road test has two parts to determine the applicants ability to evidence ordinary and reasonable control in the operation of a motor vehicle under the supervision of an examining officer. Ohio Administrative Code Section 4501:1-1-10

Driving Test [ + ]

• This examination consists of a test of the applicants driving skill.

• You must furnish the vehicle for your driving test and it must be in safe operating condition. The examiner will inspect your car before giving you the test.

• You will perform tasks such as:
1. Stopping and starting;
2. Turning around and backing up;
3. Making proper left and right turns;
4. Giving hand signals or using mechanical turn signals;
5. Driving in the proper lane;
6. Maintaining a safe following distance;

• NOTE: Buckle your seat belt and keep both hands on the steering wheel while testing.

  Passing Score
  Applicant will be considered to have passed this portion of the examination provided he does not receive a minus score in excess of 25 points.

Maneuverability Test [ + ]

• The maneuverability test is conducted in two steps. Each step is to be completed in one continuous motion. The examiner will instruct you to steer to the right or left of the point marker.

• Step One

  You must drive forward through a box (9' by 20') formed by four markers. Steer to the right or left of the "point" marker (20' ahead of box in center of course) as directed by the examiner. Steer to a straight ahead course and stop when the rear bumper of your car is even with the "point" marker and generally parallel with the course.

• Step Two

  From your stopped position at the end of Step One, you must be back past the "point" marker. Straighten your car and back through the box and come to a stop with your front bumper even with the two rear markers and generally parallel with the course. If you stop your vehicle because of striking a marker, you will have to go back to start and begin again (See diagram below).

• Before the test is complete, your vehicle must be removed from the test area without running over a marker or committing any other dangerous action.

• Scoring
  Points are deducted for:
  1. Stopping to check progress;
  2. Bumping markers;
  3. Misjudging stopping distance;
  4. Not parallel with test area;

  5. Immediate failure = running over or knocking down a marker or other dangerous action.

  Passing Score
  Applicant will have passed this portion of the examination provided he does not receive a minus score in excess of 25 points.
• You must take your examination passed form to a Deputy Registrar (See County Lists) to apply for your Ohio driver license. The license will expire on your fourth birthday after issuance. If you are under 21, it will expire on your 21st birthday.
